Update on new procedures at your orthodontic appointments

Everyone entering the office must wear a mask.

Please brush prior to your appointment our brushing station and restrooms along with all other common areas are closed

Every patient must have a waiver signed if under 18 a parent or legal guardian must be present to sign.

(Included on this page is a copy of the consent form for your convenience. If you can fill it out prior to the appointment that would be great.)

Every patient will be required to have their temperature taken and use the hand sanitizer provided at the front desk and when exiting the office.

No parents will be able to enter the clinic. If you have questions or concerns one of our team members will be happy to speak to you after the child’s appointment at your vehicle.

Please do not arrive any earlier then 5 minutes prior to your appointment or you may be asked to wait in your car.

If you have an exam or consult appointment one parent only will be able to accompany the patient into the consult room and will be required to have their temperature taken as well.

Please call us after the appointment to schedule for your following visit. If you have to leave a message we will return your call within 24 hours.

Please do not come to your appointments if you have been sick or running a fever within 14 days of your appointment date.

Anyone with a temperature of 100.4 or higher will be asked to reschedule.

We will have a new way to check-in via text or email, you will receive an email or text prior to your appointment please follow the directions to check-in.

Per regulations, we have implemented the following:

Patient Screening – All patients will be screened upon arrival to our office. This will
include taking their temperature and filling out a questionnaire/consent form.

Limited Number of People in Our Office Common Areas – We ask that patients only come into the office. We will have the main door for the entrance and the side door for exiting. One of our staff members will call you within 30 minutes of your appointment to schedule your following appointment.

Masking Required – We ask that everyone entering our office be masked. Please bring
your own mask to your appointment. Our staff will be masking as well.

Sanitation – All our equipment and clinical areas will continue to be thoroughly cleaned
and sanitized per current safety regulations and guidelines. We will continue with stringent practices of frequent hand washing before and after every patient along with wearing gloves, face shields, and masks.

To Our Patients Who Are Experiencing Symptoms – Please call to reschedule your appointment if you have any of the following symptoms: cough, sore throat, difficulty breathing, temperature over 100.4 degrees or muscle aches. If you have been in contact with anyone diagnosed with COVID19, we ask that you self-isolate and reschedule your appointment for 14 days post exposure or aforementioned symptoms.
